If you believe you are a Settlement Class Member in this Settlement involving certain disposable contact lenses purchased between June 1, 2013, and December 4, 2018, and you wish to apply for a settlement benefits, you must have completed a Claim Form. Claim Forms must have been completed either online or via U.S. Mail no later than March 10, 2021. Claim Forms received after this deadline may not be accepted.

To file online, you will be asked to provide information about all your qualifying contact lens purchases you made during the Class Period. This information includes date of purchase, price, and location. You will also be able to upload any documentation required for your claim including proof of authority if you are filing on behalf of the Claimant. Please have all this information ready before you start your claim. You will not be able to save your claim and finish up at a later date.

If you would like to file by mail, download a copy of the Claim Form here. Please read the form carefully, fill it out completely, and attach any documentation as applicable. Then, mail the completed Claim Form to the following address:
